We are excited to welcome your family to our school! As part of the enrollment process, you will need to complete the Enrollment Package, which includes the following forms:

1. Tuition Contract

2. Admission Agreement / Health Assessment

3. Immunization Record or Exemption Form

4. Parent Handbook Acknowledgement

Please note that the Admission Agreement / Health Assessment form must be completed online in your browser. Once finished, download the form as a PDF to your computer and then upload it using the file upload option provided within the enrollment package.

The undersigned agree to enroll my child mentioned above at Lion Academy Childcare for the above mentioned Academic Year subject to the following terms and conditions:

- **Full-Time Tuition: This option covers 45 hours per week. A fee of $20 per hour will be charged for each hour beyond 45 hours.
- **Part-Time Tuition: This option is applicable for 25 hours per week. A fee of $20 per hour will be charged for each hour exceeding 25 hours.
- **Preschool-Only Fee: This fee encompasses services from 8:30 AM to 12:00 PM. If picked up after 12:05 PM, a penalty of $10 will be applied for every additional five minutes. Failure to clock your child in or out may result in overuse fees. Should tardiness persist, the parent or guardian may be required to pay the full-time tuition rate or withdraw the child from the program.

- Option 1: The monthly tuition will be billed on the first day of each month and is due by the fifth. A late payment fee of $25 will be assessed weekly until the tuition is paid in full, including any accumulated late fees.
- Option 2: I wish to divide the monthly tuition into two payments, to be paid on the fifth and fifteenth of each month. A late fee of $25 per week will be charged if the tuition is not paid in full by the due dates.
Any additional arrangement requests must be discussed with the director. Children will be grouped and billed based on classroom placement rather than age. We aim to ensure that children are grouped with peers they will enter kindergarten with (those who will be five years old by September 1).
I agree to provide 30 days written notice of my intent to withdraw my child from Lion Academy. I acknowledge that I will be responsible for payment for thirty days if I withdraw without supplying the required notice.*
I understand that tuition is a fixed monthly amount. No discounts will be applied for missed days or holidays. (Holidays are listed in the parent handbook)*
Lion Academy will be closed during the week that includes Christmas Eve and extends to January 2. As a gesture of appreciation for the staff, I agree to pay the full tuition for December.*
Four months each year contain five weeks. Lion Academy will not impose additional charges for the fifth week, as this policy is intended to compensate for holidays and absences.*
Any account that remains overdue by more than 30 days will continue to incur a weekly late fee of $25 and may be referred to a collection agency, whereby I agree to assume responsibility for all associated collection and legal fees.*
I will be responsible for initiating a new tuition agreement if my childcare hours change or when my child has a birthday that may result in a reduction or increase in tuition.*
